  • Authorization Release: An "Authorization Release" form must be completed by the every job applicant before any employment background check services can be performed.
  • Authorization Release - Continual: Once an individual is hired, you are unable to utilize the original authorization release form to perform future background checks. In accordance with the FCRA, it is necessary to secure a separate document authorizing continual background checks. This document is useful when performing post hire background checks for the purpose of promotion, reassignment, and employee retention.
  • Alaska MVR: Required to release Alaska MVR records.
  • New Hampshire MVR: Required to release New Hampshire MVR records.
  • Washington MVR: Required to release Washington MVR records.
